Why Psychological Safety Must Become the New Baseline for HSE Strategy
Nigeria wrote the cost of workplace silence into law more than a decade before Australia began building the machinery to prevent it. Neither country has closed the gap between recognising the problem and stopping it before it happens.
There is an inversion buried in the global psychological safety debate that most HSE professionals outside a handful of elite jurisdictions never get to see, because the debate itself is conducted almost entirely inside those jurisdictions. Nigeria’s Employees’ Compensation Act, enacted in 2010, was among the earliest statutes anywhere in the world to recognise mental stress as a compensable workplace harm, entitling a worker to payment when psychological injury arises from a sudden traumatic event or from work demands that unfairly exceed their capacity.
Australia did not begin building an enforceable, prevention-focused psychosocial hazard regime until 2022, and its states only completed that rollout in 2025. On paper, Nigeria moved first. In practice, the two systems have ended up mirror images of the same unfinished project. Nigeria compensates psychological harm after it occurs but has little machinery to prevent it. Australia, on the other hand, is now building the machinery to prevent it, but had to wait more than a decade after Edmondson’s research entered the mainstream to do so.
Neither gap is really about law. It is about whether the HSE function treats voice, whether a worker feels safe enough to say something is wrong before it becomes a claim, as a hazard to be managed at all. That gap needs to be examined globally rather than through the lens of the handful of countries producing the loudest psychosocial safety regulation, because the employees most exposed to the underlying problem live disproportionately outside them.
A global HSE strategy that treats psychological safety as a Northern Hemisphere compliance trend, something to be adopted once a multinational’s home regulator requires it, will miss both the scale of the problem and the most promising place to solve it cheaply.
1. Stating the Argument
For most of its professional history, HSE has organised itself around the control of physical hazards, including energy isolation, confined space entry, fall protection, permit to work systems, and the hierarchy of controls. This architecture, refined across a century of industrial regulation from the United Kingdom’s Health and Safety at Work Act to Nigeria’s own Health and Safety Code for the Nigerian Electricity Supply Industry, has produced genuine and measurable progress wherever it has been properly resourced and enforced. It has not, however, solved the problem of silence.
A technician on a distribution feeder in Lagos who observes a protection relay has been bypassed, a supervisor on a mine in Johannesburg who sees a shortcut being normalised, an engineer on a Kigali high-rise construction project who suspects a design margin has quietly eroded under commercial pressure, all sit inside systems that assume the hazard is the primary risk. The deeper and more persistent risk is behavioural: whether that person believes speaking up will be met with curiosity or with reprisal, and whether the organisational culture around them makes that calculation easy or nearly impossible.
Harvard Business School’s Amy Edmondson defined psychological safety in 1999 as a shared belief among team members that the team is safe for interpersonal risk taking, the confidence that admitting a mistake, asking a naive question, or challenging a senior colleague will not result in humiliation or retaliation. Google’s Project Aristotle later studied 180 teams over roughly two years and found that who was on a team mattered less than how team members interacted, with psychological safety emerging as the most important predictor of team effectiveness.
This finding does not belong to any single region, but its practical expression varies enormously depending on how hierarchical the surrounding culture is, how formalised the employment relationship is, and how much regulatory or financial pressure exists to act on it, three variables on which Nigeria, and much of Sub-Saharan Africa, sit at a genuinely different point than Australia, the United Kingdom, or the United States. The conventional assumption inside most HSE functions, that psychological safety is relevant to innovation in a technology company but not to the prevention of serious injury on a construction site in Kogi State, is the mistake this article sets out to correct.
2. The Reality Beneath the Numbers
The behavioural evidence base is now difficult to ignore wherever it has been measured. Research published by Occupational Health & Safety (OH&S) on January 15, 2026, found that 80–90% of serious workplace injuries trace back to human error or unsafe behavioural choices, with some estimates placing the share of all workplace accidents beginning with unsafe behaviour as high as 96%.
Stress alone was found to reduce the likelihood of a worker choosing the safe course of action by roughly ten percent, and fatigue compounded the effect, with about half of employees admitting to serious mistakes when tired or distracted. Nigerian research on the same question, though far thinner and less funded than its Northern equivalents, points the same direction. Studies on mental health in the Nigerian workplace estimate that between thirty and fifty percent of employees experience some form of work-related mental health problem, and construction sector research has found direct relationships between environment, workload, role clarity, and near-miss safety outcomes, exactly the psychosocial variables ISO 45003 asks organisations to assess.
The honest complication, and the one most global HSE strategies fail to name, is that Africa’s occupational health data is thin precisely where it is needed most. The ILO’s 2025 review of national occupational safety and health statistics identified persistent gaps in data availability, coverage and comparability, including underreporting of occupational accidents, limited surveillance of occupational diseases, and uneven coverage of workplace hazard exposure. A parallel review of South African industry statistics found the available occupational disease and injury data to be unstructured and inconsistently reported, symptomatic of the absence of a functional national surveillance system.
Chronic stress, informality, and job insecurity degrade judgment, suppress reporting, and reduce the visibility of harm all at once. Where the workforce is largely informal, as across most of Sub-Saharan Africa, there is frequently no incident register at all to reveal the problem. Where the workforce is formal but hierarchical, as in much of corporate Nigeria and South Africa, incidents may be recorded, but the near misses and early warnings that would allow prevention are disproportionately unlikely to be voiced. Either way, the organisation is flying blind, and psychological safety is the variable that determines how blind.
3. The Category Error, and Why It Plays Out Differently by Region
There is a second, more subtle failure running through current global practice, and it takes a different shape depending on where an organisation sits. In wealthier markets, the error is treating Employee Assistance Programmes, resilience training, and stress awareness campaigns as equivalent to psychological safety. These are commendable interventions, but they address an individual’s capacity to cope with an unsafe environment, not whether the environment itself allows the truth to surface. ISO 45003:2021, the first global standard addressing psychological health and safety at work, makes this distinction explicit by directing organisations toward managing psychosocial risk at the level of work design, leadership behaviour, and organisational systems, rather than defaulting to individual coping resources.
In Nigeria and much of the wider region, the same category error takes a slightly different form: the assumption that because the Employees’ Compensation Act 2010 already provides for mental stress compensation, and because ISO 45001 certification is already the aspiration of the most sophisticated Nigerian organisations, psychosocial risk is therefore adequately covered. But it is not. Compensation activates after harm has already occurred and been proven; ISO 45001 governs physical and procedural risk far more thoroughly than it governs the psychosocial and cultural conditions that determine whether a hazard gets reported at all. To be fair to ISO 45001, it addresses this in Clause 5.4 (Consultation and Participation of Workers), which requires leadership to provide an environment that empowers workers to voice concerns without fear of reprisal. How many lead auditors dig deep enough to establish this as a structural audit finding is a question that deserves an honest answer: not many.
Nigeria has a legal category for the consequence of psychological harm without a widely adopted management system for its prevention, the exact inverse of what Australia has just spent three years building. South Africa’s Department of Employment and Labour has begun to close this gap directly, with anticipated amendments to the Occupational Health and Safety Act of 1993 expected to introduce explicit requirements for employers to assess and manage psychosocial hazards, alongside a broader move, formalised through 2025 regulations on physical agents and general safety, toward requiring a genuine integrated health and safety management system rather than a written policy alone.
There is a third, region-specific dimension a genuinely global analysis cannot avoid. Hofstede’s cross-cultural research scores Nigeria at 80 on power distance, a measure of how much a society accepts an unequal distribution of authority, well above the global average and close to the scores recorded across much of West and East Africa. That cultural pattern is a structural fact that any psychological safety strategy in this region must design around rather than import wholesale from flatter, lower power distance cultures such as the Netherlands or Australia, where speaking up to a superior carries a fundamentally different social cost.

The strongest empirical case for treating psychological safety as core HSE infrastructure, rather than a cultural nicety, comes from decades of research into high reliability organisations, industries such as commercial aviation, nuclear power generation, and offshore oil and gas that operate complex, tightly coupled systems where failure can be catastrophic and irreversible, and that now span every region including West Africa’s own offshore fields and Nigeria’s national grid. The organisations that sustain the strongest safety records are not the ones with the fewest reported problems. They are the ones with cultures that actively surface small problems constantly, treating every near miss and every voiced concern as free information about a system one step away from failure.
Commercial aviation’s adoption of Crew Resource Management (CRM) provides one of the clearest historical examples of how concerns about hierarchy and psychological barriers to challenging authority can be translated into formal organisational practice. Following a series of major accidents, including United Airlines Flight 173 and the Tenerife disaster, aviation authorities and airlines increasingly recognised the dangers of overly hierarchical cockpit cultures, where junior crew members were reluctant to challenge captains. United Airlines introduced the first comprehensive CRM programme in 1981, and CRM subsequently became embedded in airline training and regulatory requirements worldwide.
There is a genuine tension here that an honest global analysis has to name rather than gloss over, and it is sharper in high power distance economies than in the flatter cultures where psychological safety research originated. Building a culture where junior staff routinely challenge senior staff can, if poorly implemented and stripped of its cultural context, blur into a loss of authority and slower decision making in situations that genuinely require speed and clear command, and it can also be experienced as culturally alien or disrespectful in societies where deference to seniority carries deep social meaning beyond the workplace. Edmondson herself has been careful to distinguish psychological safety from mere niceness or comfort; the goal is candour under pressure, not the elimination of hierarchy.
The workable target for organisations across Nigeria and similar cultures is narrower than a wholesale import of flat, Silicon Valley style openness: a culture where challenge on matters of safety specifically is explicitly carved out as an exception to normal deference, clearly sanctioned by the most senior leader present, so that raising a safety concern is not experienced as disrespect but as an expected part of the job regardless of rank.
5. What Decision Makers Consistently Overlook
Three specific blind spots recur across organisations operating in Nigeria and similar emerging markets, and each carries strategic weight beyond the safety function itself.

Development finance institutions, including the International Finance Corporation and the African Development Bank, increasingly apply Environmental and Social Performance Standards to project finance across Africa’s power, telecoms, and infrastructure sectors, standards that already require assessment of labour and working conditions, including psychosocial elements, as a condition of funding. For a Nigerian DisCo, a data centre operator, or an infrastructure developer seeking DFI-backed capital, organisations that treat psychosocial risk management as optional are quietly pricing themselves out of that capital pool.
Without a direct measure of voice, validated psychological safety surveys, stop work authority utilisation, or near-miss to recordable ratios tracked over time, and without deliberately extending that measurement into the informal supply chain, organisations cannot see the gap that international disasters across multiple high hazard industries have repeatedly shown to be preceded by internal warnings that were raised, documented, and ignored.
6. Why This Matters Now, Strategically and Economically
For ESG and OHS professionals working across Nigeria and the wider region, the strategic stakes have shifted from reputational to financial and operational faster than domestic regulation has moved. The Australian and South African regulatory reforms function as an early template regardless of where an organisation is headquartered, because global capital markets, insurers, and DFIs read regulatory direction in leading jurisdictions as a signal of where underwriting and lending standards are heading everywhere else. A Nigerian organisation exporting oil and gas services, aviation support, or power sector expertise into markets that have already legislated psychosocial hazard management will increasingly be assessed against that standard by counterparties and insurers, whether or not Nigerian law has caught up.
The economic case sits alongside the regulatory and financial ones. Recent workplace research indicates that higher psychological safety is associated with lower burnout and lower intentions to leave, suggesting that psychologically safer workplaces may support employee retention. These facts matter enormously in a Nigerian labour market where skilled engineers, safety professionals, and technicians are highly mobile regionally and internationally. Turnover and burnout are themselves leading indicators of incident risk; new and fatigued workers, and teams destabilised by high turnover, are disproportionately represented in serious injury statistics across nearly every high hazard sector.
7. Practical Steps HSE and ESG Professionals Can Take Now
Several actions require no new budget cycle and are directly adapted to resource-constrained, high power distance, and informally structured markets.
The global safety profession has spent more than a century learning how to control the physical causes of harm. It has been slower to understand and manage the psychology of harm, the conditions that determine whether people feel safe enough to speak up before an unsafe decision leads to an incident.
Nigeria recognised the cost of silence in law more than a decade before Australia began developing stronger systems to address it, yet neither country has completely closed the gap between recognising the problem and preventing it. Every HSE professional in Nigeria and across the region should therefore ask a simple but uncomfortable question: if a junior technician discovered tomorrow that a senior engineer had made a wrong decision, would the technician feel safe enough to speak up, and would anything positive happen to them for doing so?
The answer to that question may say more about the true safety of an organisation than its incident rate, audit score, or compensation claims. A genuinely safe organisation is not simply one where accidents are rare. It is one where people feel safe enough to speak when something is wrong, and where speaking up leads to action rather than punishment.
